• Oasis charting, guidelines, and regulations. Strict adherence to QR and team case conference in accordance to patient status, progress, and POC. Responsible for a very large territory upon hire, due to staffing. Travel radius exceeded 200 miles ranging through 7+ counties. As an RN, responsible for completing regular weekly or multi weekly home visits, new patient admissions, recertifications, discharges, pm visits, on call visits, wound care, wound measurements and coordination in healing process in a timely manner with MD. IV insertion, maintenance and lab draws, catheter initiation and care, patient and caregiver education, med reviews and med teaching. Problem lists including diabetic, COPD, wound, PVD, CAD teaching, surgical aftercare among many others. Daily team call, afterhours charting and POC updates, supply reorders, MD calls, coordination with PCM, ED, BOM, and regional consultants in order to provide quality and DSS guided care in the home setting. Training to LPN staff provided on as needed basis as well as oversight of LPN care to patients.

• (fulltime and PRN) Responsible for caseload of max 6-7 patients of different acuity levels for medical surgical diagnoses/complications/acute care/post op. Included a variety of pediatric patients ranging from infant to teenage years. Skill set for bedside procedures such as intermittent catheterizations, lumbar puncture, injections, sterile field, blood transfusions, TPN, Gtube, PEG tube, NGT, CBI, IV insertion, etc. Ability to administer medications as well as ordered treatments and daily wound care. Post op policy includes regular interval vital sign monitoring, pain assessments, incision monitoring, specific post op ordered aftercare to meet dc criteria, and time management of patient needs and emergencies. Several competencies acquired via Healthstream academy, in person staff training, and clinical setting check off.

• MDS coordinator- daily assessments, including fall, braden, and bims scores. Filed and followed insurance claims and reviews. Priority focused on correct diagnosis coding in order to receive accurate Medicare reimbursement, show improvement in patient status according to POC, regular medication reviews, weight and infusion/IV med tracking, immunization tracking, infection tracking, weekly care plan meetings with patients and families to stay up to date on diagnosis, patient meds, needs, and improvements/deteriorations.

• As Resident Care Director, I was responsible for oversight of day to day resident care, safe staffing ratios for resident care and satisfaction, medication administration, orders and compliance, coordination with Hospice and Home Health agencies, med cart audits, resident charts, new resident contracts, Medicare assessments, change of condition assessments, immunization tracking of residents and staff, clinical policies and procedures adherence in accordance to state mandated guidelines, and many other duties subcategorized under the above. I provided support and education to staff in order to maintain compliance and safe resident outcomes in accordance with DSS standards and expectations. Successful completion of ISP and UAI certifications, state survey, and Healthcare Oversights. I actively ran indeed ads for hiring process and followed candidates through orientation and stay interviews for staffing census. Attended resident meal times for resident satisfaction, safety, and quality. Attended resident council meetings for 5 star ratings and continuation of improvement in quality of care.

• SDC- Clinical orientation of new hires, responsible for vaccines (Covid and Flu), PPDs upon hire, orientation and annual competencies for direct care staff, monthly (or more) inservice for all staff, CNA class coordinator, CPR instruction, monitoring courses completed on healthcare academy learning modules, education to direct care staff whenever needed or observing trends needing improvement. QA weekly/monthly audits of medication administrations, treatments, MAR/TAR documentation, PCC reports, monthly QAPI meetings, COR meetings monthly, care plans, infection control reporting, processes, and policies. Report to ADON and DON directly on a daily basis.

• Carilion Children's Pediatric hospital has been a very wonderful learning experience upon graduation as a novice nurse. I have been privileged to learn very intimately and in an “attentive to detail” way in aspects of a large age range of children and many different health conditions, both chronic and acute. I start IV therapy for both fluid and medications/infusions, I perform lab tests, apply oxygen, obtain vitals, assess patient progress/deterioration in order to effectively and quickly communicate among the healthcare team to ensure patient safety and goals are reached. I am able to work with a team or by myself on many occasions. I work with other units for transfers and new admits. I can locate supplies for doctors and medical professionals. I am able help train new health care workers in order to keep our unit running smoothly and safely.
