I explained Advance Care Planning to patients who were referred by their physicians for this service. If the patients decided to complete an Advance Medical Directive, I assisted them with this process. I worked remotely the majority of the time as the job took place during the pandemic, however, I also met with patients face to face if they requested that.

I attended orientation and skills check off for the 2021 flu season, but resigned prior to the beginning of that season as I was offered another position which I could perform remotely.

• Provided discharge instructions to patients per provider's instructions, which could include education regarding medication and lifestyle changes, scheduling of tests ordered by provider, explanation regarding application for financial assistance and other various programs patient was recommended for
• Performed role of charge nurse at the Care A Van clinic which includes coordinating and overseeing operation of entire clinic, triaging medical situations to determine best resolution (referral to ED or to see provider at clinic)
• Reviewed children's vaccine records from the USA and also many other countries to determine what vaccines patients need to attend school, and also to offer other recommended vaccines appropriate for patient's age. Administered vaccines, to both children and adults
• Occasionally filled in for the roles of intake and laboratory
• Attended training for Advance Care Planning facilitator, and have performed this role with a few patients during the last year.

• Provided hospice care to patients in their homes. Care included assessments of patients' comfort, reporting patients' needs to their doctor, administering needed medication for comfort care, providing education and support to family caregivers, referring to social workers and/or chaplains as needs were identified
• Performed the role of supervisor of a hospice team (RN's, social workers, CNA's, chaplains) for eighteen months. This included making assignments to each team member and overseeing the overall care provided to patients

• Provided comprehensive nursing care to patients on medical-surgical floor of small hospital. Care included physical assessment, medication administration and reporting of results, accurate documentation of all treatment administered, report to physicians of any areas of concern, following all standards of nursing care
