● Completes initial face-to-face assessment for every admitted member to identify discharge needs within 24 hours of admission. ● Active participant in daily Care Without Delay rounds reporting on patient progression towards the established discharge plan. ● Document in KPHC and communicate the assessment outcomes to determine the appropriate transition plan with MAPMG physician healthcare team and patient/family. ● Send referrals/communicate with in-network vendors for coordination of post-acute levels of care such as Home Health, DME, IV infusion, SNF, Sub-Acute and Acute Rehab. ● Document any updates, care progression and barriers to discharge daily and as indicated on assigned patient. ● Manage timeliness of care progression with physician and nursing staff to prevent avoidable delays and or days. ● Assure follow up appointments and referrals to ambulatory case manager for high-risk patient population are scheduled and communicated to patient/family prior to discharge. ● Collaborate with Social Worker to coordinate, long-term care, assisted living, financial assistance and other services required.

● Provides acute inpatient hemodialysis and peritoneal dialysis care and treatment, including developing initial and ongoing assessments, and providing patient training and education as applicable. ● Responsible for providing communication with the physician and other members of the health care team to interpret, adjust and complete all care plans in a timely manner. ● Assisting and supporting the acute unit management team in achieving high standards for quality, patient satisfaction, staff development, business management as appropriate, teamwork culture and employee satisfaction.

Routinely observe and guide direct patient care staff for appropriate technique and adherence to facility policies and procedures. ● Promote and assist with compliance to OSHA programs in order to maintain a safe and clean working environment. ● Maintain overall shift operation in a safe, efficient, and effective matter. ● With Facility Administrator conduct staff meetings at least monthly or as needed to keep patient care staff informed of changes in patient care needs or operations to improve delivery of care. ● Meet routinely with the Clinical Manager to discuss personnel and patient care status, issues, and information. ● Supervise all documentation of patient information. ● Assess daily patient care needs and develop and distribute patient care assignments appropriately. ● Assume primary responsibility in an emergency situation. ● Assess patient needs, respond to dialysis treatments, and communicate concerns to rounding physician. Implement changes in patient care/treatment as directed. ● Monitor and supervise all patient care activity during dialysis and assist as necessary. ● Collaborate with the direct patient care team in making decisions to benefit patient care. ● Continuously monitor the patient's condition with regards to problems and potential complications associated with dialysis. ● Administer medications to patients per physician’s orders.

Collect specimens according to established procedures. This includes, but not limited to: drug screens, biometric screening and insurance exams. ● Administer oral solutions according to established training. ● Research test/client information and confirm and verify all written and electronic orders by utilizing lab technology systems or directory of services. ● Responsible for completing all data entry requirements accurately including data entry of patient registration; entry of test order from requisition or pulling order from database; managing Standing Orders. ● Enter billing information and collect payments when required, including the safeguarding of assets and credit card information. ● Data entry and processing specimens including: labeling, centrifuging, splitting, and freezing specimens as required by test order.

Takes vital signs, performs weights, feeds patients, empty foley bags/drains and performs or assists with activities of daily living. ● Verifies patient identity and explains specifics of testing procedures to patients before performing phlebotomy or ECG. Maintains a log of specimens and diagnostic ECGs. Prepares laboratory and ECG or x-ray test requisitions as requested by the RN. ● Performs specific diagnostic test and treatments under the direction of the RN to include foley catheter insertion and discontinuation; appropriate casting and splinting and crutch walking. Other patient care responsibilities are suture set-ups and simple dressing changes and suture removal. The Nursing Tech II also performs blood glucose monitoring and removal of IV catheters. ● Monitor cardiac rhythm. Documents and mounts strips per protocol. Notifies professional nursing staff/physicians of life threatening rhythms or any changes in the rhythm. ● Documents data collected and care given in a clear and concise manner. Presents documentation for RN reviews and signature. ● Maintains sterile instruments, portable oxygen and equipment. ● Safely transports patients, specimens and equipment. Demonstrates an awareness of safety issues

● Takes responsibility for the direct delivery of professional and competent patient care by promoting quality care standards. ● Acts as Charge Nurse/ Control Drug Nurse at the direction of the Head Nurse in setting and monitoring standards for patient care. ● Performs triage to determine the priority of patients’ treatments based on the severity of their condition. ● Participates in the monitoring of the planning, delivery of care and evaluation of individualized patient care. ● Participates in the care, custody and administration of medicines in accordance with the hospital’s policies, procedures and guidelines. ● Ensures that all nursing documentation is completed accurately and understands the importance and relevance of their confidential nature. ● Ensures that infection control practices are adhered to in accordance with the infection control policies. ● Uses equipment and supplies in cost effective and responsible manner and participates in the procedure for ordering supplies within the agreed budget as delegated.

Works directly under physicians, assisting them during exams, diagnostic testing and treatments such as joint injections and aspirations. ● Assists physicians with splinting, plaster cast and back slab application, removal and after care. ● Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications. ● Cleans and bandages wounds. Performs phlebotomy procedure. ● Provide care for patients suffering from various bone problems and give health teaching regarding some prevention measures for orthopedic conditions and diseases. ● Deals with skeletal conditions such as post-op joint replacement, arthritis, fractures, and osteoporosis. ● Assess how a certain orthopedic problem affects patients with their activities of daily living and help patients meet the challenges that they are facing. ● Assists the orthopedic physicians during minor operations or examinations. Provides direct care during recovery and treatment.

Gives direct nursing care. ● Identifies, plans, gives, and evaluates nursing care. ● Admits, and discharges patients. ● Performs such nursing procedures such as IV therapy, oxygen therapy, dressing changes and blood drawing. Performs irrigation, catheterizations, and gives enemas. ● Assists patients in mourning, maintaining proper alignment of the patient's body and utilizing good body mechanics. ● Take and record vital signs. Observe medical asepsis. ● Observe signs and symptoms, institutes remedial action when appropriate and records these in his chart. ● Prepares patients for, and assists physicians with, diagnostics and therapeutic procedures, and when examining patients. ● Carries out doctor's legal orders. Charts accurately medications and treatments given. ● Administers and charts medications and patients reactions to medications. ● Performs post mortem care.
